Mission Statement
Our mission is to provide comprehensive coordinated care to individuals with life limiting illness and to their families and to support them in the process of death and bereavement.

Philosophy of Care
A patient with life limiting illness has the right to live his/her final days comfortably, to the fullest extent possible, and to experience a dignified and peaceful death.

Has over 20 years of healthcare experience. Graduated Texas A&M B.S., Highest Honors, Baylor College of Medicine, Doctorate of Medicine, Surgical Resident and Conroe Medical Education Foundation Family Medicine Resident, received outstanding 2nd year Resident Award. Board Certified Family Medicine Dedicated to medically supervising patient’s and family’s decision to remain at home or in a home like setting in life’s final stages.

Carmen T. Green, RN, BSN, MBA Administrator/Nurse Supervisor
Has over 27 years of critical care nursing and supervisory experience. Extensive experience with end of life issues. Life long resident of the Clear Lake Area. Devoted to providing exemplary patient care.
